Consent and agreement
I have read and understand the "Summary Plan and Agreements" document which was emailed to me by DS Coaching. I understand and agree that I participate in the activities prescribed to me under the guidance DS Coaching entirely at my own risk. I have considered the nature of the activities prescribed and I am satisfied that I am sufficiently responsible and competent to assume full and entire responsibility for my own safety whilst under the supervision of the coach and also whilst undertaking activities independently. I am aware that I must abide by the instructions of DS Coaching or any centre staff where I undertake the prescribed activities. I will carry out the sessions with care and abide by the rules of the coach and the centre (if applicable).Agree
Payments and Cancellation
I understand that I am agreeing to a minimum three month period of coaching with DS Coaching at the rates agreed in an email received by me. I also agree to giving one month's notice if I wish to cancel after the initial three month period expires. If I am attending any face to face sessions I further agree to DS Coaching's cancellation policy for such sessions which is available for me to view at https://www.dscoaching.co.uk/cancellation-policy/Agree
